This Personal Information is obtained in multiple ways including correspondence, by email, by telephone, via our website, from publicly available sources, and from third parties.

We collect your Personal Information for the primary purpose of providing our services to you. When we collect Personal Information, where appropriate, we will explain to you why we are collecting the information and how we plan to use it.

Most of the information will be stored in client files of our practice management software which will be kept for a minimum of seven years.

You are able to access Personal Information we hold about you and to update and/or correct it, subject to certain exceptions. In the event you wish to access your Personal Information, please contact us along with confirmation in writing. For security purposes, we will require identification from you before releasing the requested Personal Information.

Complete Motion Physiotherapy will not charge any fee for you access request but may charge a reasonable administrative fee for providing a copy of your Personal Information.

Maintaining the Quality of your Personal Information

Complete Motion Physiotherapy is committed to keeping your Personal Information up to date and make sure that it is accurate, complete, and up-to-date. If you find that the information we have is not up to date or inaccurate, please advise us as soon as possible so we can update our records and ensure we can continue to provide quality services to you.
